How to access OSP

There are two ways to participate in OSP West:

1) you can ask your primary care provider (e.g. family physician, nurse practitioner) or mental health provider (e.g., psychiatrist) to submit a referral on your behalf, or

2) you can submit a self-referral for the program yourself. Referrals can take up to 4-6 weeks to process and are processed in the order in which we receive them.

Before submitting a referral, please review the “Information Sheet” to see if this program is a good fit for you. Importantly, are you willing to partner on your treatment and actively participate in the following ways?

  1. Ask Your Primary Care Provider to Submit a Referral:

You can ask your primary care provider to submit a referral form through their Electronic Medical Record (if available), or complete this online referral form. Unsure how to speak to your doctor about your mental health? See our handout “Talking to Your Doctor About Mental Health.”

 

  1. Submit a Self-Referral:

If you would prefer to submit a self-referral, please complete our online referral form by clicking the button below. You may also have another care partner assist you with completing the online referral form. If you would like to submit a self-referral, but do not feel comfortable with the online process, please call us at 905-387-8361 or toll-free at 1-833-944-9966, email OSPwest@stjoes.ca, or speak with your primary care provider. This referral form is for adults ages 18+. If you are between the ages 15-17, please speak with your doctor or visit the BounceBack Ontario website for information about services for youth.

OSP is not a crisis or emergency mental health service. If you require immediate support, please refer to the appropriate crisis service.

 

OSP provides support for adults* (aged 18+) with depression, anxiety, and anxiety-related concerns, including:

  • Depression and low mood
  • Generalized anxiety and worry
  • Social anxiety and performance fears
  • Unexpected panic attacks and agoraphobic fears
  • Health anxiety
  • Obsessive-Compulsive concerns
  • Post-traumatic stress
  • Specific fears
  • Other anxiety and stress related problems (e.g. work stress, test anxiety)

These services are available to residents in the South and West regions of Ontario, including

  • Windsor Essex,
  • Chatham Kent,
  • Sarnia Lambton,
  • Oxford,
  • Elgin,
  • London Middlesex,
  • the Kitchener Waterloo area,
  • Cambridge,
  • Guelph Wellington,
  • Huron Perth,
  • Brantford Brant Norfolk,
  • the greater Hamilton area,
  • Niagara,
  • Burlington, and
  • Grey Bruce.
